The Affordable Care Act (ACA) of 2010 extends health insurance coverage to millions of Americans. Many states offer health insurance plans through exchanges, or online marketplaces where consumers can shop for plans that best meet their needs. In California, the exchange is called Covered California. Californians in need of health insurance can visit or call 1-888-975-1142 to enroll in coverage.

All health insurance plans purchased through Covered California must cover the following 10 essential health services:

  1. Ambulatory patient services
  2. Emergency services
  3. Hospitalization
  4. Maternity and newborn care
  5. Mental health and substance abuse services
  6. Prescription drugs
  7. Rehabilitative services
  8. Laboratory services
  9. Preventive and wellness services
  10. Pediatric services, including dental and vision care

Individuals and families with incomes below 400% of the Federal Poverty Level (FPL) may qualify for tax credits to help pay the costs of health insurance. These credits are only found at Covered California. In addition, the Medi-Cal program is being expanded to cover more Californians. You can determine if you are eligible for subsidized health insurance by visiting


The purpose of Medi-Cal is to provide comprehensive medical services to children and adults in low-income families. Single adults and families who are eligible but have income levels above prescribed levels must pay or obligate their excess income toward the costs of their medical care.

How to Apply

Obtain a Medi-Cal application from any one of the offices listed on our Facilities page, or phone the Department of Social Services at (559) 675-7670 and request to apply for Medi-Cal. You may also click on the following link to complete an application and mail to the following address: P. O. Box 569, Madera, CA 93639.

Your application will be screened and assigned to an Eligibility Worker who will determine your eligibility for the Medi-Cal program.

You have the choice to meet with the Eligibility Worker for a face-to-face interview or complete an interactive interview by phone or you can return your forms and verifications all by mail.
