12 diseases: Polio, Diphteria, Tetanus (lockjaw),
Pertussis (whooping cought), Measles, Mumps, Rubella (German

Varicella (chicken pox), and Pneumococcal infections.
Clinic Times
Madera: Every Wednesday 8:00 - 10:00am and the 2nd and
last Wednesday 4:00pm - 6:30pm.
Chowchilla: 1st Wednesday of the month 2:00 - 4:00pm.
Oakhurst: 4th Friday, 9:00am - Noon

Preventive Health Care for Adults (PHCA)
The PHCA program provides FREE health assessments, screenings,
education, counseling and referrals to persons 50 and
over to help them to achieve and maintain their highest
level of health and independence. Comprehensive health
screenings and counseling are provided by public health
nurses.

assessing the incidence of and preventing the spread of a wide
range of infectious diseases in the community.

Health Nursing
Provides in the areas of maternal and children's health, chronic

Tobacco
Control and Education Program
Provides technical assistance to school districts, health
organizations, governments, and private citizens on
health issues regarding tobacco prevention and environmental
tobacco smoke.
Vital
Statistics
Issues certified copies of birth and death certificates,
authorizes burial permits, registers births and deaths
and out-of-hospital births, acts as a liaison between
the public and the State of California Office of Vital
Records, compiles data relating to births, deaths and
infectious diseases reported in the county, and provides
statistical information upon request.
West
Nile Virus
West Nile virus (WNV) is an illness that can
be spread to humans through mosquito bites. Symptoms of
WNV are similar to flu-like symptoms and can include fever,
severe headache, and body aches.
