What is CHDP?
The Child Health and Disability Prevention (CHDP) Program
is a preventive health program under Children's Medical
Services (CMS), serving children and youth. CHDP makes
regular well-child checkups available to eligible children
– not only those with obvious health problems, but
also those who appear well.
The Child Health and Disability Prevention (CHDP) Program
is a Federal and State mandated health care program for
infants, children, teen, and young adults. The goals of
the program are to:
- Encourage families to seek preventive health care
- Identify and prevent health problems
- Link people with treatment, education, support services
and comprehensive healthcare coverage
Health problems detected early can help prevent future problems
with:
- Growth and development
- Learning difficulties
- Time off from work for parents
- Serious health complications
Who is eligible for CHDP services?
- Medi-Cal clients from birth through 20 years of age
- Low to moderate income clients from birth through
18 years of age
- Children in Foster Care
What services does CHDP provide?
CHDP offers complete preventive health checkups at NO
COST to the family. The health check-up includes:
- A complete medical and developmental history
- An unclothed examination
- A dental screening (contact local program for a list of dentists in your area)
- A nutritional assessment
- Vision and hearing screening
- Immunizations
- Urine, blood, and TB screening tests
- Certain lab tests as needed
- Tobacco education
- Health education and anticipatory guidance
Who provides these exams?
The CHDP health exams are provided by CHDP approved:
- Private medical doctors, clinics, medical groups and HMO's
- Comprehensive health centers and some hospital clinics
How does one obtain a CHDP exam?
What is the CHDP Gateway?
CHDP Gateway is a process that provides eligible applicants for temporary full-scope Medi-Cal Health coverage. It also makes it easier to apply for Medi-Cal and Healthy Families.
Who is eligible for the CHDP Gateway?
Those children whose family’s income is at or below 200% of the poverty level, who are California residents, have no insurance, and are between the ages of 0 through 18 years old may be eligible for temporary full-scope Medi-Cal through CHDP Gateway process.
How does the Gateway benefit children?
Those who are eligible will receive temporary full-scope Medi-Cal for the month of the CHDP exam plus an additional month. During this time period, a child is eligible for the following health services:
- Dental care
- Doctor visits
- Specialty doctor visit and treatment
- Prescriptions
- Vision care
- Hospital services
- X-ray and laboratory tests.
In order for your child to continue coverage beyond the temporary full-scope Medi-Cal period, the parent must fill out the Medi-Cal/Healthy Families application that he/she will receive in the mail. This must be completed before the ending date of the temporary full-scope Medi-Cal period. If done in a timely manner, health coverage will continue for the child until Medi-Cal/Healthy Families makes a final eligibility determination.

What is the cost?
It depends on the number of children in your family, your
household income and the insurance plan you choose.
You can have affordable monthly premiums from $4 per child
to $27 per family and $5 co-payments for some services, plus,
immunizations and checkups at no cost.
