
The PHCA program provides FREE health assessments, screenings,
education, counseling and referrals to persons 50 and over
to help them to achieve and maintain their highest level
of health and independence. Comprehensive health screenings
and counseling are provided by public health nurses.
What is the purpose of the PHCA Program?
Through individualized health care plans, participants
identify health problems, find needed health services, and
learn preventive health practices. Providing this type of
support helps participants change their habits and behaviors
so they may live healthier, more fulfilling lives and avoid
the cost of chronic disease.
What does the PHCA Program include?
A yearly comprehensive health assessment which includes:
- Health history and health survey
- Measurement of pulse, blood pressure, height and weight
- Annual fall influenza immunization
- Tetanus and pneumonia immunizations
- Nutrition, medication and health habits review
- Individualized
counseling, referral and follow up
- Preventive health education presentations
- Community health
promotion

Where is the PHCA Program located?
Health services are provided at various senior sites, churches,
housing/apartment complexes and activity centers throughout
the county in Coarsegold, Madera, Chowchilla, Madera Ranchos,
Raymond, Oakhurst, & North Fork.

* The Preventive Health Care for the Adults Program is sponsored
by the County of Madera, Fresno/Madera Area Agency on Aging
and the California Department of Public Health Services.
*Note: In 2002, the PHCA program was honored by the U.S.
Department of Health and Human Services’ Administration
on Aging as a model senior health promotion program. The
Strategic Plan for an Aging California Population identifies
the program as an investment that helps participants avoid
costly, acute primary care and long-term support expenditures.
