GENERAL NOTES:
1) Owner / applicant has secured Registered Professionals (Civil Engineers and Land Surveyors) by the State of California in the design and layout of the road for construction to the prescribed width, slopes and grades as required by the County of Madera.
2) Registered Professional has used standard practices in their profession in providing services to the owner / applicant in the preparation of the plans necessary to construct the road to the Caltrans Highway Design Manual (HDM) or the American Association of State Highway and Transportation Officials (AASHTO) publication “A Policy on Geometric Design of Highways and Streets” latest editions.
3) Contractor(s) shall comply with all Federal, State and Local Ordinances, Regulations or Laws as related to their performance of the work necessary to establish the prescribed work as outlines by these plans and the Engineer of Record specifications.
4) Contractor(s) shall resolve any conflict between the plans and existing site conditions with the Engineer of Record and the County of Madera, Road Department.
5) Construction shall be performed to general construction practices, these plans, and the latest edition of the California State Standard Plans and Specifications along with latest edition of the County of Madera Standard Specifications.
ENGINEER’S NOTES:
1) Engineer of Record shall be responsible for establishing Best Management Practices (BMP) in compliance to the Clean Water Act (CWA) Section 402 National Pollutant Discharge Elimination System (NPDES) for the project. 2) Engineer of Record shall be responsible for certifying that the earthwork does meet the compaction requirements per the standard specifications.
3) Engineer of Record shall submit a certification that the road was constructed in accordance to these plans, his / her specifications and to the standard specifications prior to its acceptance. The certification shall include evidence that the earthwork meets or exceeds compaction requirements to the standard specification, that the road geometry was constructed to the plans and the standard specifications, that the aggregate base or road surface road aggregate is to the standard specifications, that the asphalt concrete meets or exceeds the standard specifications and that the signs and striping are in place to standard specifications.
SURVEYOR’S NOTES:
1) Surveyor shall set the necessary construction stakes for the earthwork contractor to establish the necessary roadway widths, slopes and grades in accordance to the plans and Engineer of Record Specifications and / or standard specifications.
2) Surveyor shall verify that the earthwork contractor has constructed the roadway to the plans prior to the placement of sub base material or driving surface material, i.e. class II aggregate base (AB) and so inform the Engineer of Record, which will include a statement to that effect in his / her certification to the County.
CONTRACTOR’S NOTES:
1) Contractor shall be responsible for the implementation of the Best Management Practices (BMP) in compliance to the Clean Water Act (CWA) Section 402 National Pollutant Discharge Elimination System (NPDES) for the project. 2) Contractor shall apply for and receive an encroachment permit from the County of Madera Road Department prior to commencement of work in County of Madera Right of Way. Encroachment permits may be obtained at 2037 W. Cleveland Avenue, Madera, California 93637-8420 or by contacting Gary Bispham at (559) 661-6333 Ext.504.
3) Contractor shall comply with the Manual on Uniform Traffic Control Devices for Streets and Highways (MUTCD) 2003 Edition (or the latest edition) with the California Supplement. 4) Contractor shall be responsible for locating all underground utilities prior to the start of any work by contacting Underground Service Alert (USA) 48 hours prior to any excavation at 1-800-227-2600
5) Contractor shall be responsible for contacting the appropriate party in advance of any work for necessary inspections in compliance to these plans, standard plans and standard specifications.
6) Earthwork contractor is responsible for contacting the Engineer of Record prior to the earthwork process for clarification on how compaction test are to be performed in compliance to these plans and the required specifications.
7) Earthwork contractor is responsible for establishing the necessary roadway widths, slopes and grades in accordance to the construction stakes set by the Surveyor of Record and prior to the placement of sub base material or driving surface material, i.e. class II aggregate base (AB).
8) Earthwork contractor is responsible for placement of erosion control measures to meet the Clean Water Act Section 402 National Pollutant Discharge Elimination System as directed by the Engineer of Record.
CONTRACTOR’S NOTES: Continued:
9) Asphalt concrete (AC) paving contractor is responsible for verifying with the Engineer of Record and / or the Surveyor of Record that the earthwork, and base material has been placed as outlined on the plans and specifications. If in the opinion of the paving contractor, he / she finds that the widths, slopes and grades are not in accordance to these plans then it is his / her responsibility to contact the Engineer of Record and / or Surveyor of Record for clarification on how to proceed prior to the placement of the pavement.
TRAFFIC NOTES:
1) All permanent required traffic control devices, including street signs, shall be in place and accepted by the County of Madera prior to allowing any public traffic onto any portion of the road(s) being improved by these plans.
2) All traffic signals, street lights and appurtenance thereto shall be reviewed and approved by the County of Madera Road Department prior to their installation, including manufacture certifications in accordance to the standard specifications.
3) All traffic signals, street lights and appurtenance thereto shall be field inspected an approved by the County of Madera Road Department prior to their being energized. The contractor shall take appropriate actions to clarify to the traveling public that traffic signals are non-operative prior to them being energized.
